How do I check the status of my referral?
Health Net Federal Services is the managed care support contractor for the TRICARE West Region. In order for you to see your referral, you must set up an account online at
www.tricare-west.com
** You must set up an online account to see your referrals or call HealthNet @ 844-866-WEST to request a hard copy.
Can I change my Primary Care Manager?
Yes, you can change your Primary Care Manager at any time. To change your PCM, please call Health Net at 1-844-866-9378 or visit the Health Net Federal Services website at
www.tricare-west.com
How do I find a Tricare Network Specialty Provider?
Call Health Net at 1-844-866-9378 or visit their website at
www.tricare-west.com
and search the provider directory.
Is there a TRICARE Handbook?
No, but you can find information on all Tricare programs at
www.tricare-west.com
.
How do I get a copy of my medical records?
You can stop by the Medical Records office, sign a release of information (
DD2870
) and request copies of your medical records. You can also request that the PAD/Medical Records office send copies of your records to outside specialists by following the same process. Call 520-533-2507 for more information and assistance.
How do I get care if I’m TDY or not permanent party?
Bring a copy of your orders with ID card and complete a quick registration with a receptionist at the RWBAHC main entrance front desk.
What are Advanced Directives?
Advanced Directives are decisions patients make in advance that clearly state what they would like done in the event of an irreversible or terminal illness. Patients at RWBAHC who would like to make Advance Directives need to visit the post JAG office at 2387 Hatfield Road, Bldg 51102, Ft. Huachuca, AZ 85613
**Advanced Directives are not necessary at RWBAHC because if a “life event” occurs, the patient will be transferred by ambulance to the closest Emergency Room.
What if there is incorrect medical information in my medical record?
If you believe that a provider documented incorrectly into your medical record, the first step to getting the issue corrected is to contact your provider and request they amend your record.
If the provider does not amend your record and you feel it is still incorrect, you can fill out RWBAHC Form 429, Patient Request for Amendment of Protected Health Information form and turn it in to the RWBAHC HIPAA Privacy Office.
